Description: QuantiaCare is a digital health platform that engages and empowers healthcare consumers with instant access to content, solutions, and data to improve health outcomes. It offers a suite of mobile app products designed to help track and manage health and wellness goals by providing interactive diaries, multimedia presentations, practical tools, quizzes, community discussions, and tips from nationally recognized clinicians.
Objectives: Provide accessible mobile patient education and digital tools for condition management.

MyDMJournal
A powerful and intuitive app to help you track and analyze your blood glucose, exercise, insulin, weight, and nutrition. Find 60k pharmacies and 800k clinicians. Share reports via email or fax with your provider. Time your exercises in the background to get honest minute counts and start-times for exercising. Calculate your BMI. Analyze data by trend or time of day and share reports with your healthcare provider. The app features interactive diabetes education from nationally recognized clinicians. 
EatSmart
EatSmart demystifies diet and nutrition for people at risk for or with diabetes by breaking the topic of eating into concise and digestible slices with quick and practical tips you will want to use right away. Developed with the nationally-recognized dietitian and diabetes educator Hope Warshaw, MMSc, RD, CDE, the app offers simple meal planning tips and tricks. The continually updated content is easy to consume as teachings are delivered in bite-sized pieces in an interactive mobile viewing format, including voice, text, easy-to-understand graphics and images, and engaging questions and answers.
HUMC Diabetes
This unique curriculum is designed for patients and their families, introducing a wide variety of topics that are essential to proper diabetes management. Each topic is covered in 10 minutes or less: Calculating your Insulin Bolus, Understanding Body Weight, Dining Out with Diabetes, Traveling with Diabetes, Diabetes and Depression, Identifying and Managing Hypoglycemia, Using Glucagon for Severe Hypoglycemia, Diabetes and a Trip to the Hospital. The app also includes a weight diary and BMI calculator, a pharmacy and health professional locator, and the ability to fax or email summary reports to a selected physician.
DailyCoach
There are lots of ways to be a more active person. DailyCoach, which helps users find engaging ways to increase their exercise, is designed for people who have diabetes or are at risk of acquiring it and patients with cardiovascular conditions or obesity issues. Gary Scheiner, MS, CDE reveals brief insights into finding exercise routines you'll actually try. Learn and then share your experiences with others. Short, one- to three-minute segments teach users how to set goals, track progress, and reward themselves. Other segments focus on exercise safety and routines for various settings, such as home, work, and commuting.

DiabetesIQ
Participate in quizzes with extensive feedback, comparing results with other contestants as they progress through multiple game levels by solving visual puzzles. Diaries let you track your glucose, blood pressure, weight, and exercise, and you can share trends with a health professional or caregiver. DiabetesIQ was created by the collaborative efforts of the University of California, San Francisco Diabetes Teaching Center and QuantiaMD. The app is an innovative new way for patients to explore their knowledge of the forms, progression, treatment, and complications of diabetes.
